Demographics: approximate population – 49,300, 96% White, 2% Black/African American,1% Asian and <.05% other, median age – 35.8, 21% under the age of 18, 4% of individuals 5 yrs of age and older speak a language other than english at home, meadian income – $47,151 (14% of households earn less than $15,000 annually)

Goals

  • Increase and Expand meaningful participation of stakeholders including youth and family involvement
  • Facilitate Youth Mental Health First Aide training / Trauma Informed Care training
  • Support local family engagement activities
  • Improve hospital discharge planning process
  • Collaborate with other NYS counties that have been successful doing similar SOC work

Overview

Cortland County is one 12 Upstate NY counties that volunteered in July 2011 to take part in a pilot project funded by NYS OMH. In October 2012 these same 12 counties became the first cohort to participate in the NYS Success System of Care (SOC) Learning Collaborative. The Cortland County SOC team originally set out to develop a local system of care that would provide a safe, competent and timely response to children (and their families) who were being referred for services through their Single Point of Access (SPOA) Committee. This plan was altered due to changes in infrastructure for service delivery and financing including federal and state system changes for child/adolescent behavioral healthservices. The SOC team decided to merge with the existing Tier 2 committee and to focus on the goals listed above.

Cultural & Linguistic Competence

  • 9/27/13 – 3 hr Cultural Competence Workshop facilitated by NYS Success consultant Lenora Reid-Rose. Workshop was NYS OASAS certified for CASAC credit.

Family

  • Supported the development of the “Parent’s & Change” support group

Youth

  • Supported the development of a “Youth Hub”, an online media platform that will be used to help further engage youth

  • 3 Youth Mental Health First Aide training sessions held
